Individual Plans

  • 18 years & under: $49

  • 19-39 years: $69

  • 40-64 years: $79

  • 65+ years: $89

Family Plan

  • Family of 4: $219

    • Includes 2 adults and 2 children under age 19

    • Each additional child under age 19 can be added for $29/mo

*One time enrollment fee of $50 with membership.

No contracts. We do ask that you provide 30 days notice of cancellation.
If you choose to re-enroll after cancellation there is a $100 re-enrollment fee.

  • Acute care/Sick Visit/Consult $150 per visit
