Our vision

We strive to redefine what healthcare feels like — where every patient is seen, heard, and supported on their unique health journey. Our vision is a healthier community built through education, connection, and personalized care that promotes lifelong wellness, not just symptom management.

Our goal is to provide individualized, high-quality healthcare that honors the whole person — blending traditional and functional medicine to meet each patient where they are and guide them toward lasting health, improved quality of life, and empowered self-care. We listen deeply, treat compassionately, and create personalized care plans rooted in science, guided by empathy, and aligned with each patient’s unique goals.

If you are a patient who is looking for healthcare tailored to you have found the right place.

We’re also partnering with local businesses to bring the benefits of Direct Primary Care to their teams. If you're an employer looking to offer your staff accessible, relationship-based healthcare without the red tape—we’d love to connect.

What is dpc?

Direct Primary Care is healthcare without the middleman. Care can be focused on you, the patient, instead of checking boxes and codes for insurance. It allows you direct access to your provider, more time with your provider without feeling rushed, a care plan and decisions made by you and your provider (not an insurance company). Instead of billing insurance, you pay a low monthly membership. No copays, no surprise bills. Compassionate, personalized care aimed to meet your goals.

Kimberly Blake, APRN, FNP-C

I graduated from the University of Central Arkansas in 2007 with a Bachelor of Science in Nursing. While working as a registered nurse I decided to continue my education. I graduated in 2010 with a Master of Science in Nursing from the University of Central Arkansas.

With over 17 years of experience as a Registered Nurse and Advanced Practice Registered Nurse, I’ve had the privilege of caring for patients across nearly every stage of life and in a wide variety of clinical settings — from neonatal intensive care and pediatrics to primary care, wound care, nursing homes, urgent care and the emergency department.

Throughout my career, one thing has remained constant: my deep belief that every patient deserves to be heard, understood, and treated as a whole person — not just a set of symptoms. One of my goals is to create a space where personalized, high-quality care is not rushed, where we meet patients where they are, and where wellness is a journey we walk together.

I’m currently pursuing certification in Functional Medicine through the Institute for Functional Medicine, expanding my ability to identify and treat the root causes of illness — using a thoughtful blend of traditional and holistic approaches, tailored to each patient’s comfort, goals and lifestyle.

I believe in making changes sustainable for the patient, informed choices and empowering patients with the knowledge and support they need to improve their health, longevity, and overall quality of life. I care. I listen. I’m here to help you feel your best — and if I can’t help directly, I’ll make sure you get to someone who can.
