What is not included in your membership:

This list is not all-inclusive. If for some reason, something is not covered in your membership, you will be made aware of this upfront. We make every effort to avoid any surprise costs.

• Medical imaging & Lab work (annual wellness labs are included; additional labs are offered at significantly discounted rates, insurance/HSA/FSA can be utilized for these where applicable)

• Surgery or procedure performed outside of our office

• Cost of immunizations (these can be ordered/given from local pharmacy and are often covered by your insurance).

• Obstetrical care and delivery

• Durable Medical Equipment (crutches, walkers, immobilizers/splints, etc.)

• Prescription & Injectable medications (In-office medications are offered at significantly discounted rates, minimal

• costs)

• Hospitalization or ER/urgent care visits

• Specialty consults (we do have access to several specialists available for consult at no additional fee)

** Every effort is made to avoid any additional costs outside of your membership fee. Generally we can provide the majority of your care. If something arises outside of our capability we have access to various specialists for consult and are building referral networks. We will continue to build a network of affordable resources.
